We are seeking an experienced leader for the position of Assistant Vice President of Care Management (AVPCM). This position will provide support to the Vice President of Care Management and be jointly responsible for the oversight, structure, and support for the Care Design NY’s CCO/HH and HCBS Care Managers that work with the member served by CDNY.
It is critical that the AVPCM understand; demonstrate in their performance; and lead the department to ensure that:
The AVPCM will educate and respond to the respective community which it serves in all ways to ensure individuals are obtaining services in all CDNY regions. The position is also responsible for ensuring compliance with all local, state, and federal regulations, as well as organizational policies. The individual will work in cooperation with other departments, including but not limited to the Fiscal Department, Human Resources, and Quality Assurance/Compliance to ensure that the organization is well managed in its entirety.
The successful candidate will have experience in, and a proficient understanding of, the Medicaid Service Coordination model of service provision, as well as how to skillfully and successfully transition that service to the new Care Coordination/Health Home model of services for people with intellectual and developmental disabilities.

We are seeking several individuals for the position of Regional Directors of Care Management (RDCM). This position will oversee the operations of a geographic area supporting the care management services to approximately 2500 individuals and support and promote the mission and values of CDNY for Health Home Care Management and HCBS Care Coordination.

The RDCM is responsible for the direct support for the care manager directors, regional health care manager, intake coordinator, and regional office support staff operating in one of the six CDNY regions. The regional director will support all operational staff in their region, while helping to create an environment and culture that promotes the values of CDNY. The position is also responsible for ensuring compliance with all state and federal regulations, as well as organizational policies. The Vice President of Care Management (VPCM) is a key leadership position within the organization. Reporting to the COO, this position represents the most senior position in Care Management. The VPCM will support and promote the mission and values of CDNY for HH Care Management and HCBS Care Coordination in all counties served by the CCO.

The VPCM will fully support the culture of CDNY through the development of care management staff, continuously directing initiatives and operating procedures, which lead towards enhanced skills and job satisfaction. The VPCM will fully support methods and technology that provide for the highest quality of care management and person-centered services for members. The position is also responsible for ensuring compliance with all state and federal regulations, as well as organization policies. The individual will work in cooperation with other departments, including but not limited to the fiscal department, human resources and quality assurance, to ensure that the organization is well managed in its entirety. Care Design New York has regional offices in Albany and Manhattan; this position is based in the Manhattan office. Travel is primarily local during the business day, although some out-of-area and overnight travel will be expected.

The successful candidate will possess a Master’s Degree in a related field required along with ten years of experience in the field of IDD. Ten years of prior supervisory experience with five years at the senior management/executive level is preferred.
