Our care coordination services are available in your community, in your primary language, and with a sensitivity to your religious and cultural background.
A Care Manager works with a network of providers to plan and manage the services and supports you want and need. Your Care Manager wants to know what is important to you, what you need help with, and what your goals are.
Your Care Design NY Care Manager is also aware that things change in your life, and the supports and services you receive need to be responsive to those changes. Our Care Managers are knowledgeable, professional, respectful and good listeners.

In many areas across NY State, individuals most impacted by the COVID-19 epidemic require necessary access to services, resources or providers critical to their health and well being. The Provider Relations team at Care Design NY developed the COVID-19 Search of Service ("SOS") tool for Care Design NY Care Managers containing information around resources that can push-in or support Care Design NY members and families during this critical and unprecedented time.
In July and August of 2019, Care Design NY held managed care focus groups in four regions throughout New York State (NYS), including Long Island, Manhattan, Hudson Valley and Capital District/ North Country. The Focus Groups were facilitated by a neutral third-party contractor.
