Care Design NY's 5-Year Anniversary

Dear Members, Families, and Friends,

Today is a special day-- Care Design NY’s 5-year Anniversary.  We invite you to join us in celebrating this milestone!

When Care Design NY launched its Care Coordination Organization/Health Home (CCO/HH) five years ago, we served 25,000 individuals with Intellectual and Developmental Disabilities (IDD). Today, the team supports over 28,000 members and families across 30 counties in New York. We are very proud of the CCO/HH we created in 2018 to fulfill this mission:

Care Design NY will empower individuals with Intellectual and Developmental Disabilities (IDD) to live their most fulfilling lives as members of their communities with support from a person-centered, culturally competent workforce, and a diverse provider community.

Our Care Coordination Organization/Health Home for individuals with IDD provides specialized services to coordinate care, combining developmental disability services and supports with health, wellness, and behavioral health services. Members are benefiting from the extensive care coordination infrastructure we developed that includes medical, behavioral, and mental health expertise. We are a quality improvement focused organization and are continuously evaluating quality health metrics. Individuals with IDD are at the highest risk of being excluded from society. Each day, the Care Design NY team embraces diversity, equity, inclusion, and belonging (DEIB), along with a dedicated DEIB department that facilitates the growth, development, and integrated practices into our everyday business operations. Our dedicated care managers are removing barriers to achieve acceptance and inclusion for the members and families we support so they may live quality lives. Weaving the threads of equity and justice into everything we do is core to our mission and vision.

We are unified under the common goal of assisting individuals with IDD to live their best lives.  Members and families are at the center of everything we do!  On behalf of the entire Care Design NY team, thank you for trusting in us to provide person-centered care coordination services.
