We work with health care professionals, disability service organizations, community agencies and others so that children and adults with Intellectual and Developmental Disabilities (IDD) receive the services and supports they need to live their best life.

Care Design NY is a nonprofit Care Coordination Organization/Health Home (CCO/HH) certified by the NYS Office for People with Developmental Disabilities (OPWDD), providing specialized Care Management services to children and adults with Intellectual and Developmental Disabilities (I/DD) to help them live a quality life.
