We work with health care professionals, disability service organizations, community agencies and others so that children and adults with Intellectual and Developmental Disabilities (IDD) receive the services and supports they need to live their best life.
Care Design NY is a nonprofit Care Coordination Organization/Health Home (CCO/HH) certified by the NYS Office for People with Developmental Disabilities (OPWDD)
Care Design NY provides specialized Care Management services to children and adults with Intellectual and Developmental Disabilities (IDD) to help them live a quality life.
Individuals we support receive a dedicated Care Manager who oversees and coordinates their access to services, based on their plan of care, goals, and desired outcomes. Care Manager responsibilities include, but are not limited to, support with:
Developing an individualized “Life Plan” that documents personal goals, needs for services and supports, and identifies the resources that will help meet their goals.

- Care Coordination: streamlined communications make it easier for you to access information
- Value Based Care: improving outcomes through coordinated services not only improves quality of care for individuals but also bolsters organizations value in a managed care environment
- Assistance with Compliance/Regulatory Considerations: our care managers help coordinate follow-up care and work with individuals to comply with provider recommendations
- Referrals: as part of an integrated network, care managers will have access to partner organizations for referrals
