Administrative and Executive support for Care Design NY is provided by our sister organization, Partnership Solutions, a Management Services Organization (MSO).
Care Design NY is a diverse team of over 1000 professionals dedicated to one thing: helping individuals with Intellectual and Developmental Disabilities live their best lives. We provide care management services for the Care Coordination Organization/Health Home (CCO/HH).
