Care Design NY is partnering with new providers every day. 

We are committed to building a robust and comprehensive network, and to assisting our partners in managing the 31,000 (and growing) individuals affected by intellectual and/or developmental disabilities, and their complex health and wellness needs.

Network Collaboration

Care Design NY (CDNY) is a NYS-certified Care Coordination Organization and Health Home (CCO/HH) exclusively serving people with Intellectual and/or Developmental Disabilities (I/DD) and those who care for, and about, them. Acknowledging the unique needs of the individuals we support, CDNY seeks partners providing services to the I/DD community (including but not limited to medical, behavioral, therapeutic, habilitative, developmental disabilities, social and community supports) to collaborate with CDNY and support the work of its Care Managers and help fulfill the Life Plan goals of its members.

Collaboration Goals

Care Coordination Organization (CCO) and Network Collaborative Partner (Health Care Provider, IDD Provider Agency, or Community-Based Organization) will work together in the following areas:

  • Open Communication

    • CCO and Network Collaborative Partner to inform each other of any member-related issues in a timely manner
    • Respond timely to information sent by either party with the goal of addressing any provider and/or member concerns

  • Care Coordination

    • Whenever possible, Network Collaborative Partner to incorporate CDNY Care Managers into provider workflows
    • CDNY Care Managers to assist with adherence to plan of care and upcoming appointments
    • Collaborate with CDNY’s clinical staff

  • Service Access

    • Identify and address service gaps
    • Reduce barriers to accessing care
    • Referrals to appropriate services
    • Network Collaborative Partner to inform CDNY of program openings or newly implemented programs to prompt referrals for services
    • Assist members in receiving and maintaining access to benefits (Medicaid, SSI, SNAP, etc.)
    • Be available to members to discuss questions or concerns regarding their care

  • Quality Outcomes

    • Person-centered planning
    • Preventive care monitoring, including annual physicals/well visits, dental exams, preventive care screenings, and flu vaccinations
    • Chronic disease management
    • Reduce ER and Inpatient hospital stays, including readmissions
    • Avoidable hospitalizations
    • Transitions in care and discharge planning
