CARE DESIGN NEW YORK is a New York State (NYS) approved specialized Care Coordination Organization (CCO) developed by more than 70 agencies in 30 counties of NYS who came together to support more than 25,000 individuals with intellectual and/or developmental disabilities and their families in their local communities. To achieve our mission of assisting individuals with Intellectual and/or Developmental Disabilities to get the supports and services they need to live a quality life, we are building an integrated network of organizations that will support the individuals we serve.
The Role of Care Management
Care Design NY coordinates support services for individuals with intellectual and developmental disabilities by connecting them with our network of partners. New York State has recently expanded our role to include health care, wellness, and community services in addition to those specifically targeted to people with developmental disabilities.
By developing a Life Plan with each individual, we are able to coordinate person-centered services that respond to their changing needs. Care Design NY is building a robust network of partner organizations in each community to meet the needs of each individual we serve and streamline communication between agencies.
