Sharing Expertise at the NYS ID/DD Nurses Association Annual Conference

Care Design New York was proud to be represented at the 2025 New York State ID/DD Nurses Association (NYSIDDNA) Annual Conference, held in Albany on September 17. Brittany Miske, MBA, RN, AGNP-BC, LSSGB, our Vice President of Healthcare Management and Clinical Support, delivered an insightful presentation titled Hospital Transition of Care Pearls to a statewide audience of nursing professionals.

The presentation focused on improving health outcomes and care coordination for individuals with I/DD during and after hospital stays. Key topics included:

  • Frequent hospitalizations and the patterns behind them

  • Challenges tied to premature discharge, including risks of incomplete treatment and care delays

  • Chronic disease trajectories, such as COPD, dementia, and congestive heart failure, with strategies for supporting individuals throughout their care journey

  • System-level opportunities to improve transitions through better communication, stronger hospital partnerships, and proactive care planning

  • Tips for safe discharge planning, with a focus on collaboration and person-centered care

  • Insights on supporting residential transitions, including best practices for navigating in-person and virtual IRA screenings
