Why is this important for me or my loved one?
Being enrolled in Health Home Care Management means that your Care Manager will work with you and your providers to get you the best care to meet your needs and, if needed, advocate on your behalf. In order to support you, your Care Manager needs to access your:

Annual physical, dental and other medical and mental health documents.

Vaccination & other preventive care records.

Appointment dates, as well as all hospital admission and discharge information.
Remember that your Care Manager...
Will conduct an assessment to understand your medical, behavioral, and social needs, which are then incorporated into
your Life Plan.
Has your consent to request health records and access health-related alerts in order to assist in coordinating with your providers, social service agencies, and care givers.
Should receive copies of your health records. These records can come from your providers and/or can be shared with your Care Manager at any time by you or your loved one.
Will provide ongoing outpatient management of your routine medical care and coordinate care across specialists and providers to achieve safer and more effective health outcomes.
