Pop-up COVID-19 Vaccination Clinics | BRONX

Individuals enrolled in care coordination/health home services by Care Design NY in the Bronx and Staten Island regions are eligible to receive their vaccine. Appointment only. Contact the Care Design NY Care Manager.

• 1st Dose: March 16, 2021 Clinic Hours: 10 am - 4:30 pm By appointment only.
• 2nd Dose: April 13, 2021 Clinic Hours: 10 am - 4:30 pm By appointment only.

This I/DD-specialized clinic is designed to remove barriers for individuals with I/DD. Their unique physical and sensory needs will be taken into consideration to inform the trained personnel who will be supporting them Saturday. Extra time will be allowed between vaccinations to make sure everything is fully explained, answered, and understood, using a person-centered approach with clear information for each step of the COVID-19 vaccination process.
