Eligibility & Enrollment Event

Care Design NY is here to help you each step of the way through the enrollment process. Our Enrollment Specialist will be your guide in compiling the needed documents for OPWDD eligibility and level of care. Once determined OPWDD eligible, our benefits specialist will assist with obtaining Medicaid and the enrollment specialist will explain and assist with the needed consent/enrollment forms. Enrollment happens on the first of each month, so as soon as you are enrolled you will be linked with your dedicated Care Manager.
