The Spotlight on Excellence Nominations

National Care Manager Recognition week is dedicated to showing Care Managers how much we value and appreciate their dedication, commitment, and passion in the hard work they do each day. In recognition of National Care Manager Week October 13, 2019 - October 19, 2019. CDNY is accepting nominations for a new SPOTLIGHT ON EXCELLENCE AWARD PROGRAM.

CDNY employees go above and beyond to make a difference in the lives of people we support every day. This past year, hundreds of Shout-Outs have been shared to recognize the exceptional performance and team work of CDNY employees and to thank them for a job well done.

We are pleased to have the opportunity to further recognize the talent and dedication of our Care Managers and other CDNY employees through a nomination process which includes both individual employee awards and two team awards.

Criteria for the Spotlight on Excellence Award

Employees or Teams nominated must demonstrate the core values of CDNY and the following characteristics:

  • Displays a job performance that goes beyond expected performance levels (see Core Expectations in the What to Expect of your Care Manager document attached)
  • Exhibits a person-centered approach putting the people we support front and center in all they do
  • Is motivated and strives for excellence
  • Demonstrates a sense of responsibility for their actions
  • Contributes to the positive culture and sense of community in the department and organization
  • Team awards should demonstrate an exceptional accomplishment achieved by a group of CDNY employees “going above and beyond” by working together

All nominations should provide detail on how this CDNY employee has made a difference in the life of a person we support, made a positive contribution that has impacted the lives of people we support, or made a positive impact on the organization.

Be as detailed as possible and provide specific examples of how they uphold the CDNY mission and values.  Nominations will be accepted up through October 4th, 2019.

Spotlight on Excellence Categories:

Individual Care Manager: Each region will have two Award Winners from the Care Management team.

Support Departments: A total of two awards for employees who work in CDNY departments which support care management.

CM Team Award: A total of two CM Team Awards will be given (one Upstate and one Downstate).

All entries that meet the above criteria will be eligible for the final awards. Nominees will be selected at random from the candidates in each region and announced the week of October 13th, 2019.
