The Role of the Care Manager in the Health Home

The Care Manager has an active role in advocacy and communication for all individuals enrolled in the Health Home program, as well as for all individuals who are part of the Willowbrook class. Advocacy and coordination responsibilities of the Care Manager include clinical and medical issues and apply to individuals living in the community, as well as those who live in IRAs and other group homes.

Care Managers take a whole-person approach to ensuring the unique needs of the people supported by CDNY are met, including healthcare needs and goals.  Only through collaboration with providers can we achieve a true, person-centered approach.  We ask that the Care Manager receive medical recommendations, hospital discharge documents, and medical provider follow-up recommendations timely from residential staff. 

This allows the Care Manager to review the recommendations and work with residential staff to ensure the recommendations are addressed. Care Managers also share medical and clinical information with our clinical team. The Care Manager and sometimes our clinical team can be part of meetings and collaborative discussions about the person’s often complex medical and behavioral needs and can provide advocacy with providers to ensure the person’s health needs are met.
